Infinite Health Services, LLC has a dedicated team of professional licensed social workers who maintains current knowledge of community resources. Clients with behavioral and/or mental concerns receive case management services from our social workers who provide counseling and guidance in dealing with problems and difficult life situations. They keep the RN abreast of any changes or conditions for the clients and communicates with all agencies providing direct services to the client.

Infinite Health Services, LLC sees each client face to face with a visit from a Registered Nurse or a Licensed Practical Nurse to evaluate the client's medical, health, social, emotional and financial status. In addition, the nurse will assess the client's functional ability.

Infinite Health Services, LLC provides at home safety education and awareness sessions to our clients. Poor eyesight and hearing, arthritis, dementia and side effects from medications are all factors that can make a simple trip to the kitchen a potentially dangerous situation. We believe the more information and guidance we provide during our home visits are both preventive measures to keep risks at a minimum or risk free.

 Infinite Health Services, LLC takes pride in training nurses how to effectively conduct a Nurse Case Management Assessment. This service is not only offered to our staff, but to other case management agencies as well. This service involves theory, hands-on and evaluation of their skills.
